          "An Act relating to home and community-based                                                                          
          waiver services under Medicaid; relating to personal                                                                  
          care services; establishing an adult foster care home                                                                 
          license and procedures in the Department of Health                                                                    
          and Social Services; and providing for the transition                                                                 
          of severely disabled individuals from foster care to
